Monash & Sunway Alumni Games 2019

Closing Date: Friday, 20 September 2019 or when all spots are reserved.

Note:
1) For badminton, one player must be Sunway Alumni. Individual players are to complete this registration form.

2) For futsal, the team captain and minimum 3 players must be Sunway Alumni. Maximum 10 players are allowed per team. Only the team captain is required to complete this registration form. We will request team members' information after confirmation of your participation.

3) Once you have registered, the Sunway Alumni Relations team will send you the confirmation of your participation.

4) Once the confirmation is received, each participant will need to make payment (i.e., All the players in the futsal team are required to make payment).

5) Bank Account Details: Sunway University Sdn. Bhd, HSBC Bank Malaysia Berhad - 352512164101.

6) Please send the bank deposit slip to alumni@sunway.edu.my & indicate 'Payment for Sunway Alumni Games'.

7) All participants will be required to complete an indemnity form which will be sent via email.

8) Prize giving will be held on the same day after all the games are over.

9) Each winner (1st place - 3rd place) will receive a medal & Champion will get an additional gift hamper.

10) Payment will only be refunded if the game categories are cancelled due to unforeseen circumstances.
